Carers UK has produced a new factsheet which provides clear, up-to-date information for carers who are looking after someone who is coming out of hospital.

The factsheet outlines carers’ rights during the discharge procedure, the steps that should be followed when a person being cared for is discharged, and what to do if things go wrong. The information has been fully updated following the implementation of the Care Act 2014.
